I am on my second pair (first pair lasted 3+ years). They are comfortable, they never slip out of my ears, there is an extension cable for them (so I can have it short it using my armband or long with my wristband), the sound quality is great, if I don't have my ipod turned up too loud I can still hear traffic around me but still get good quality sound.
It depends though if you can tolerate the ear hook thingys (I like them cause they keep them in place - other ear buds always fall out on me) and if the silicone ear buds fit your ears (the smallest ones fit me best)

Have a look at the Philips SHS850 Extra Comfort Headphones. Extremely comfortable, and good sound quality. I also had the problem of in-ear buds falling out. This setup fixed it though. Very stable on my head. Another good feature: Wind noise on the bicycle (if you cross-train) - almost zero.

I am still using the Sennheiser MX75 Twist-to-fit earbuds. They are not THAT comfortable, but the twist-to-fit design won't let it pop out. Ever. Sound quality wise, they are VERY fit-dependent, but once you figure the best latch/pad combination, they yield an excellent sound quality.

I'm very picky when it comes to headphones. I have some Sony bud ones (Model: MDR-EX51LP ), they are great when it is cold and I am wearing a hat, they stay in and are very comfortable.
I also have some Phillips ear clip ones that stay in pretty well, but are not that comfortable inside the ear.
I recently bought some Nike Flight Sport Strap Headphones from Target for $20 and personally hated them, I always felt that they were going to fall out, perhaps I just have ears that are to small.
Today I bought some Nike Vapor 3-Way Sport Headphones for $25 and have just come back from my run and overall am very happy with them. Comfortable and didn't feel like they were going to fall out and for the price I think good value.
